No Fault Auto - Patient pays for first visit (receipt
given for reimbursement). Remainder visits billed directly to insurance
company.
Worker's Compensation - Written authorization from
insurance company required. All visits billed to insurance company.
MVP Trivantage - Patient pays $30 for each hour of massage (up to 2 hours each
visit). Remainder is billed to Preferred Care. 12 visits per
year. TriVantage/Healthy Alternative Plans only.
MVP Health Dollars - Patient pays for visit and
is given receipt to submit to Preferred Care.
Aetna - Patient pays $40 for each hour visit. No receipt is necessary
to submit.
Flexible Spending Accounts - Patient pays for visit and
is given receipt to submit to Flexible Spending. FSA Card can also be used to make payment for automatic deduction.
